Highlights
e Health Center at LCH employs the nurse-managed
model of health care, emphasizing preventive and
community based care and fostering active
partnerships with patients, allowing them to become
managers of their own care.
In the fall of 2010, LCH received certification as a
Level 1 Physician-Connections Patient-Centered
Medical Home by the National Committee on Quality
Assurance for our implementation of evidence-based
practices and commitment to building long term
collaborative relationships with patients.
Community Services staff provided assistance to
1,434 individuals, responding to every kind of need
from simple information to help resolving a family
crisis to coaching clients through the process of
increasing family and financial stability. Services
were augmented through collaborations with
Kennett Area Community Services, Mision Santa
Maria, and Domestic Violence Center of Chester
County and throughvolunteer legal services offered
by attorney JohnWinicov.
Education continues to be the best strategy for
LCH clients to improve their employability and
capacity for financial and social success. ESL,
GED, computer and citizenship classes enable
clients to become more valuable employees,
better able to support their children’s education
and more engaged in their community.
